INSTRUCTIONS FOR WORKING PERMITS

PART I

Fill out Part 1 of the Application for Employment Certificate. Make sure your PARENT or GUARDIAN signs at the bottom of Part 1.

PART II

Fill in your DATE OF BIRTH. You must bring a photo I.D. with you when applying for your permit. Your school ID will be acceptable for Identification. NYS permit/driver’s license or current passport will verify your date of birth as well.

PART III

CERTIFICATE OF PHYSICAL FITNESS:

Form # AT-16 must be filled out by your doctor stating that you have had a physical exam less than 12 months ago and that you are physically qualified for lawful employment. This form must be signed by Doctor and contain stamp of the practice.

OR

A physical exam completed within 12 months on file in the Health Office is acceptable

Once all forms are completed, please bring them to the Health Office so that the permit can be issued. Once issued, please make copies of the permit. Do NOT let employer keep the original permit. Show them your permit and give them a photocopy for their records.

ALL APPLICANTS MUST APPEAR IN PERSON TO SIGN THE PERMIT CARD IN FRONT OF ISSUING OFFICIAL
